* Affected users have correct email addresses in their profiles.
* Notification settings are enabled globally and at the user level.

* No pattern in user role, department, or browser.

I've seen this exact scenario before. When everything else checks out, I'd start looking at your email service logs for those specific users. Some corporate spam filters silently drop automated reminders if the "from" address isn't whitelisted, even while other notification types get through.
Your hunch about API-driven tasks is a good one. In one case, we found reminders only failed for tasks created via API when the `reminder_sent` field was incorrectly set to true during creation. That one was a headache.
