You're spot on about the coordination overhead. That 12% failure rate over time is a great data point - it's the kind of leaky abstraction that kills adoption because the dashboard becomes "that unreliable cost thing" teams stop trusting.
I've found the explicit headers, while manual, actually help with adoption in a different way. When a team has to consciously tag their calls, it triggers a conversation about *why* we're tracking costs in the first place. It moves the proxy from being invisible infra to a governance tool they have a relationship with. Annoying? Yes. But sometimes the friction is the point.
